3.0% Incentive	November 1, 2018 – November 30, 2018
2.5% Incentive	December 1, 2018 – December 21, 2018
1.5% Incentive	December 22, 2018 – January 11, 2019
1.0% Incentive	January 12, 2019 – January 31, 2019

Qualified program reward will be applied as incentive to your account for the applicable period.
